✓ CT images and thrombus are real (FUMPE) ⚠ Clinical story, vitals and labs are illustrative The same CT is paired with different clinical scenarios, because the category depends on physiology, not clot burden.

Recommendations by category

Class of recommendation as summarised from the 2026 AHA/ACC/ACCP/ACEP/CHEST/SCAI/SHM/SIR/SVM/SVN guideline (Creager et al., Circulation 2026). 1 recommended · 2a reasonable · 2b may be considered / uncertain · 3 no benefit or harm · blank = no category-specific recommendation listed.

Source discrepancy: the Medscape recommendation table lists systemic thrombolysis as 3: Harm for A1–C2 and 2b (uncertain) for C3, while the same summary's bullet list says harm for A–C1 and "uncertain" for C2. This app follows the table rows. Check against the full guideline text before teaching this row.

        Catheter-based therapy

        Thrombectomy and catheter-directed thrombolysis devices

Each device is drawn as a schematic 3D model inside the real pulmonary arterial lumen and saddle thrombus of FUMPE patient 026, both surface-reconstructed from the CT. Drag the deployment slider to step through the procedure. Device proportions are schematic; catheter paths are drawn, not tracked, and the entry from the IVC is schematic.

          Evidence update after the guideline

          What changed in 2026

The 2026 AHA/ACC guideline rates catheter therapy in C2–C3 as "benefit unclear" (2b). HI-PEITHO (March 2026) and PRAGUE-26 (August 2026) were published after it, and both showed fewer 7-day decompensations with CDT in selected intermediate-high-risk patients, with no mortality difference. Teach both: the guideline grade, and the newer trial data that a PERT may weigh.
